A 27-year-old electrician was brought in by ambulance. He had been working at a height of around 3m but fell from a ladder to the ground. He does not know how he landed but had severe back pain and could not get up. The paramedics lifted him and immobilized his neck. On admission he was well with no neurological symptoms in his arms or legs. He has severe low back pain to palpation and to any movement. He has normal perineal sensation and is able to pass urine normally.
