Acute basilar artery occlusion may result in median and paramedian pontine infarction and a variety of other clinical pictures. Mortality remains high in patients with acute basilar artery occlusion, but a favorable outcome is possible with recanalization. Outcome is not measurably influenced by the time between intra-arterial thrombolysis and recanalization. This chapter discusses the indications for endovascular treatment and postintervention intensive care.
