An 18-year-old woman was admitted overnight with a 2-day history of malaise, fever, and palpitations. There was no past medical history and she was taking no medication. She was unemployed and lived with her boyfriend. She smoked 20 cigarettes a day, and drank approximately 20 units of alcohol per week. She admitted to occasional use of heroin and cocaine.
