A 29-year-old male photographer presented with a 1-week history of dysuria and foul smelling urine followed by a 5-day history of headache of gradual onset associated with fevers. He had generalized muscular aches and nausea. He had had casual sexual contact with a woman, not his usual partner, 2 weeks previously. He drank 10–15 units of alcohol per week, smoked 10 cigarettes per day and took occasional recreational ‘ecstasy’ but denied intravenous drug use.
