A 78-year-old man was transferred to a tertiary referral centre with acute coronary syndrome. Shortly after transfer he became unwell and developed ST elevation consistent with full thickness myocardial infarction. He was given thrombolysis with streptokinase during which she became bradycardic and hypotensive. This resolved spontaneously but afterwards he appeared confused.
