The quest to define autism in terms of its central characteristics has evolved greatly over the past fifty years, as evidenced both by changing descriptions of the syndrome in the Diagnostic and Statistical Manual of the American Psychiatric Association, and by changes in the topics most researched by investigators in the field. This commentary discusses some of the outstanding issues that must be confronted in order to attain a better understanding and definition of autism and move beyond the limitations of present diagnostic models.
