Case 22 introduces a 58-year-old man referred to the sleep disorders center for evaluation of snoring, daytime fatigue, and recurrent awakenings from sleep for the last 3 years. Medical history showed a 20 pound weight gain in recent years, diagnosis with chronic obstructive pulmonary disease (COPD) 8 years prior due to heavy smoking habits; experienced episodes of productive cough with fever yearly requiring antibiotics and short courses of corticosteroids, though he had never required mechanical ventilation. He denied shortness of breath with usual exertion.
