Monocytes and macrophages play many roles in human immunodeficiency virus (HIV) infection. Importantly, monocytes and macrophages mediate viral persistence and dissemination of virus into organs and tissues, including organs and tissues of the central nervous system. This chapter provides an overview of interactions between monocytes and the blood-brain barrier (BBB) in HIV/AIDS, and describes proinflammatory factors and virotoxins that are known to alter BBB functions and properties. The chapter also explores molecular mechanisms involved in HIV-induced BBB dysfunction, and discusses potential strategies for preventing or reducing entry of HIV, and HIV-infected cells, into the brain.
