This chapter presents a retrospective view of the AIDS epidemic, focusing on the evolution of our understanding of the central nervous system complications of HIV infection. The presentation is structured in three main historical sections: First, the early years of disease recognition and symptomatic management, a period during which morbidity and mortality were little altered by medical intervention, culminating in the introduction of antiretroviral monotherapy. Second, the current era of combination antiretroviral therapy, with its profound preventative and therapeutic effects. Third, the future, with its prospects for further refinements in prevention and therapy. The presentation provides a highly informed personal view, shaped by the author's experience, research, and clinical interests.
