HIV is associated with a relatively unique, non-inflammatory, subacute myelopathy that has been referred to pathologically as vacuolar myelopathy (VM) and clinically as HIV-associated myelopathy. Most of what we know about VM or HIV-associated myelopathy comes from studies conducted before the advent of potent antiretroviral therapy. Vacuolar myelopathy may be less common in the current treatment era, for reasons that have not been established. This chapter focuses on the pathological and clinical aspects of VM / HIV-associated myelopathy.
