HIV-positive individuals experience higher rates of all categories of mental illness compared to the general population. A wide range of factors may help explain this finding, including factors specific to the sexual minority status of gay men, factors associated with injection drug use, the stress of serious physical illness, and the likelihood that persons with severe mental illness may engage in high-risk sexual or drug-related behavior more frequently than those without mental illness. This chapter discusses the impact of psychiatric disorders and substance abuse on the course of HIV infection and disease. Specific psychiatric disorders covered include major depressive disorder, anxiety disorders, psychotic disorders, mania, and substance abuse, including the abuse of alcohol. The impact of aging is also considered.
