• Ageing has profound effects on cardiovascular structure and function and predisposes individuals to development of chronic heart failure (CHF). • In the UK, 89% of all CHF deaths and 74% of all CHF admisions occur in those individuals over 75 years of age.p • In older individuals CHF is more often multifactorial. CHF occurs when the heart’s pumping capacity is impaired, either because of a structural, functional, or electrical abnormality of the heart. • Multiple comorbidities and polypharmacy make elderly patients with CHF a particular challenge best approached using a multidisciplinary team approach. • In many cases the emphasis may shift from extension of life expectancy to one of improving quality of life. End of life management issues should be borne in mind and palliative care.
