Follow-up of acute coronary syndrome patients is a key role for primary care. Funding is in place through the Quality and Outcomes Framework (QOF). Nurse-led clinics have been shown to be very effective. Prevention of further events will be of great importance to commissioning groups. Clear and comprehensive discharge information is essential, together with good communication lines between primary and secondary care. The National Institute for Health and Clinical Excellence (NICE) guidelines provide an excellent resource.
