A 62-year-old man, with a history of pulmonary sarcoidosis, attended with progressive breathlessness over the preceding 6 months, such that current exercise tolerance was only 50 yards. In addition he had noticed increasing ankle swelling over the last 4 weeks. He denied any other respiratory or systemic symptoms and was a non-smoker.
