A 60-year-old man with known sero-positive rheumatoid arthritis, controlled with weekly methotrexate and non-steroidal anti-inflammatory medication, presented with a left-sided pleural effusion discovered incidentally on a CXR prior to an inguinal hernia repair. He had no respiratory symptoms. Thoracic CT confirmed presence of a pleural effusion but no other abnormality, with normal appearing pleural surfaces. Pleural aspiration yielded slightly cloudy fluid, analysis of which is documented in Table 27.1 and illustrated in Fig. 27.1.
