An 80-year-old woman was admitted to A&E with breathlessness. She had initially presented to her GP with a dry cough, and was diagnosed with a viral upper respiratory tract infection. Over the following three weeks she experienced progressive shortness of breath on exertion, and at presentation was breathless at rest. She denied sputum production, fevers and any constitutional symptoms.
