Preview

An 89–year-old woman presented with increasing shortness of breath over a 4-week period. This was associated with an irritable dry cough and lethargy, but no chest pain or constitutional symptoms. She had an extensive background history including ischaemic heart disease, myocardial infarction 2 years previously, left ventricular failure, mild and well-controlled asthma, and treated hypertension. She also suffered with short-term memory loss, which had been worsening for the last 2 years, and had been diagnosed as vascular dementia.
