A 54-year-old Asian man presented to the emergency department with a 3-month history of increasing dyspnoea, drenching night sweats and weight loss. This was associated with a dry cough and occasional small volume haemoptysis. He was born in Pakistan, moved to the UK 3 years previously and had visited his family in Pakistan 4 months previously. He worked in a restaurant and smoked 20 cigarettes per day.
