A 79-year-old man with type II diabetes mellitus, New York Heart Association (NYHA) class II–III heart failure, and a permanent pacemaker (PPM) presented to the emergency department (ED) with a 3-week history of increasing shortness of breath. His exercise tolerance had reduced and he also reported orthopnoea and paroxysmal nocturnal dyspnoea. He denied chest pain, palpitations, or loss of consciousness. The PPM had been implanted for complete heart block 7 years prior to this presentation.
