A 55-year-old man with chronic alcoholic liver disease presented with a 2-week history of increasing shortness of breath at rest, abdominal distension, and swollen legs. Although he regularly consumed half a bottle of vodka and 3 pints of beer a day he was asymptomatic until 2 weeks prior to this presentation. He had no other medical problems, had never smoked, and was not taking any prescribed medications.
