A 45-year-old non-smoker with a localized stage la T1N0M0 gastro-oesophageal junction carcinoma underwent oesophagectomy and partial gastrectomy with no immediate postoperative complications. Six months later he presented with sudden onset, ‘excruciating’, right-sided loin pain and macroscopic haematuria. He was normally independent but was not able to mobilize because of pain. He denied having any previous episodes of similar pain or renal stones. He was taking paracetamol 1 g and codeine 60 mg four times a day (qds) for the pain.
