A 22-year-old student was referred to the medical registrar on call. He described a 6-hour history of central chest pain radiating to the left shoulder. The constant dull ache was worse on inspiration and lying flat but eased on sitting forward. For 2 days prior to this presentation he had felt ‘like he had the flu’ and reported fatigue, mild breathlessness, myalgias, and low-grade fevers.
