A 48-year-old man was referred to the medical registrar on call with a 5-day history of gradually increasing breathlessness on minimal exertion and ankle swelling. The onset of the symptoms had coincided with his stopping smoking. Six months before, he had developed a chronic dry cough. His current exercise tolerance was less than 50 yards and he had lost over 10 kg in weight over 3 months. He had been dieting but also reported loss of appetite. Treatment with inhaled salbutamol and becotide, several courses of oral antibiotics, and furosemide (20 mg daily) had failed to improve the symptoms.
