A 30-year-old primigravida Gudjurati woman presented with increasing shortness of breath and swollen ankles. She was 14 weeks pregnant and spoke little English but denied previous medical or obstetric history. Examination revealed sinus rhythm, a loud first heart sound, an opening snap, and a mid diastolic murmur. Echocardiography confirmed moderate mitral valve (MV) stenosis (MS; valve area 1.2 cm2). She was started on frusemide, anticoagulated, and discharged home with cardiology outpatient follow-up.
