A 23-year-old Asian woman presented to the ED with a 2-hour history of palpitations that began whilst she was carrying washing up stairs. It was associated with chest tightness, ‘heaviness’ in the left arm, and presyncope. She was unable to give any more history. On examination she was drowsy, tachycardic (over 200 bpm), and had a BP of 100/60 mmHg. There were no signs of cardiac failure, but a symmetrical rash was noted on the face.
