A 42-year-old steel worker initially presented to the ED with a 3-day history of intermittent, sharp, localized, left anterior chest pain at rest. His symptoms were worse on deep inspiration and bending forwards, and were increasing in frequency. He had been working more overtime at work to pay for a new car, and had attributed his symptoms to muscle strain.
