A 19-year-old man with a history of repair of tetralogy of Fallot (TOF) was brought into the ED by ambulance. He had lost consciousness whilst walking to catch a bus with his father. He fell but was caught by his father, who lowered him to the ground. His father reported that the patient initially appeared pale but regained consciousness over about 2 minutes. There was no convulsion, urinary incontinence, or tongue biting. On waking he was mildly disorientated for a few minutes.
