A 70-year-old retired Korean mechanic presented with a 12-month history of epigastric discomfort and 5kg weight loss. A laparoscopic cholecystectomy had been carried out for multiple gallstones. The discomfort continued post-cholecystectomy. His past medical history included a seronegative arthritis and a salivary gland biopsy consistent with Sjøgren’s syndrome. The patient denied drinking alcohol and was a non-smoker. He had become jaundiced over the preceding 3 weeks.
