A 44-year-old French woman presented to the emergency department with severe, acute right upper quadrant pain, and was referred to the gastroenterology registrar on call. The pain had developed shortly after her evening meal, was constant, and radiated through to her back. She had vomited once. On specific questioning it appeared that she had had some back pain in the preceding days.
