A 76-year-old non-English speaking woman presented to the gastroenterology outpatient department with a 4-month history (through an interpreter) of nausea, decreased appetite, early satiety, and 3kg weight loss. She had mild abdominal discomfort and pancreatic surgery 25 years previously for a pancreatic abscess, but no history of diabetes or steatorrhoea. She had atrial fibrillation, for which she was on warfarin.
