A 42-year-old shop assistant with hypertension and obesity had a routine blood test, which showed a raised ALP. She was asymptomatic. She was on ramipril 5mg/day and a combined oral contraceptive pill. She had lost 10kg in 5 months, but attributed this to a diet. She did not smoke or drink. Examination, including breast and rectal examinations, was normal.
