• For raised total cholesterol (TC) levels, start with simvastatin. • If this is not sufficient, change to atorvastatin or rosuvastatin, or add ezetimibe. • For raised triglyceride (TG) or mixed lipid profiles, start with a fibrate. Add low dose statin later if need be. • For remnant (type III) hyperlipidaemia, use a fibrate. • For very high TG levels, use a fibrate and fish oil. • If high-density lipoprotein (HDL) falls, try nicotinic acid.
