A 26-year-old female graduate student was admitted to the acute admissions unit with a 12-hour history of rapid-onset fever, left-sided pleuritic chest pain, and a swollen left knee. There was no shortness of breath or haemoptysis. Her knee was hot, red, and swollen, and she was unable to weight bear. She had small tender red swellings over the dorsum of her right foot.
