A one-year-old child presented to the paediatric on-call team with a 5-week history of high spiking temperatures, weight loss, marked irritability, a widespread rash, and a limp. On examination, his heart rate was 140 beats/min and his temperature was 38.2°C. Chest examination was otherwise normal. He had hepatomegaly and symmetrical bilateral wrist swelling.
