A 50-year-old Caucasian man presented with a 4-year history of fleeting large-joint arthritis affecting the knees and ankles. In the 6 months prior to presentation, he had developed night sweats, anorexia, diarrhoea, and associated weight loss. Examination revealed widespread lymphadenopathy, moderate hepatosplenomegaly, and a pyrexia of 38°C.
