A 17-year-old male cricketer presented with a 2-month history of left-sided lower back pain. He was a right-arm fast bowler and had recently been selected to play for his county under-18 team. In addition, he played twice a week for his local club. Because of his county commitments, he had doubled the amount of overs he bowled per week. Bowling exacerbated the pain and rest relieved it. The back pain had become more intense since an episode 2 weeks previously when he had bowled at full speed and had developed excruciating lower back pain immediately afterwards.
