• Pulmonary metastasectomy for colorectal cancer is practiced widely but evidence for its effectiveness is lacking • A multidisciplinary team approach is recommended to select cases who may benefit from intervention • Patients who fall within the zone of uncertainty should be entered into a clinical trial • Surgical management should aim to remove all metastatic disease whilst preserving lung parenchyma • Five year survival following metastasectomy is over 40% in selected cases.
