A 53-year-old man is admitted from home where he was found by a neighbour. He has a history of a subarachnoid haemorrhage and surgery to clip the aneurysm. After the event he was well and discharged home, but he was left with personality changes and mood swings and subsequently lost his job. His personal life became more difficult and he became depressed. He has recently been considering suicide, and on this occasion had tried to kill himself with a nail gun and fired one nail into the medial aspect of his left eye. He was found alert, with a headache, talking, and orientated.
