A 35-year-old man presents with a sudden onset of severe headache that occurred whilst gardening. He is a previously healthy non-smoker. Of note, when aged 26 he suffered a single generalized seizure that was not investigated. In the emergency department he is alert and orientated with no photophobia or neck stiffness, and his headache is settling by the time he is reviewed after 4 hours. He has no discernible neurological deficit.
