Whilst the principles of surgical therapy are essentially the same, extra-temporal epilepsies differ from temporal lobe epilepsies in several respects. These differences impose more challenges in the localization of the epileptogenic zone. Similarly, long term surgical outcomes with resective surgery of extra-temporal epilepsies are less favourable compared to temporal lobe surgeries. In this chapter we discuss these challenges and the clinical approaches to the evaluation and resective surgery of extra-temporal epilepsies.
