This chapter describes in detail the epidemiology, presentation, and clinical evaluation of oesophageal cancer, together with the role of Barrett’s oesophagus in carcinogenesis. The histology subtypes of oesophageal adenocarcinoma and squamous cell carcinoma are described, as well as the staging classification for oesophageal cancer. Staging modalities and preoperative preparation are then discussed, as are the management algorithms for oesophageal cancer. These include neo-adjuvant chemo-radiotherapy, neo-adjuvant chemotherapy, and definitive chemo-radiotherapy for oesophageal cancer; followed by operative considerations and the surgical anatomy of the oesophagus. Various operative techniques for oesophageal cancer are described in a stepwise fashion. The operative elements of anastomosis and replacement conduits are discussed in detail, as are post-operative care and complications, and adjuvant therapy for oesophageal cancer. Finally, follow-up and surveillance protocols are discussed with palliative chemotherapy, palliative radiotherapy, and oesophageal stenting.
