This is produced and maintained up to date in collaboration by the British Medical Association and the Royal Pharmaceutical Society of Great Britain. See http://www.bnf.org/bnf/ for details. Many nations produce such lists, usually in collaboration with the pharmaceutical manufacturers, and publish frequent revisions. Other examples include the US Pharmacopeia and National Formulary (see United States Pharmacopeia) and the WHO's essential drugs and medicines list.
