An opiate rapidly and extensively metabolized to produce biologically active diphenoxylic acid (difenoxine). Its effect is due to an increase in muscle tone in the small intestine. Lomotil™, a common antidiarrhoeal drug, is a mixture of diphenoxylate hydrochloride and atropine sulphate, the latter added to prevent recreational abuse of the opiate.
