The set of information that identifies an individual as precisely as possible. The uniqueness of each individual is established by fingerprints, DNA profile, and/or iris scan. Many record linkage systems, as well as hospital and other health care record systems, including billing systems, prescription records, etc., use unique identifiers to avoid the possibility of confusing one person with others who have the same name. Identifying data usually include personal and family names, or a code representing these names, a sequence of eight digits for birth date (year, month, day), and another code for sex (male or female). Additional information may include birthplace, mother's maiden name, and father's first and last names.
