A survey instrument used to deliver a formal, carefully designed sequence of questions to the subjects of an investigation, such as a household survey of health status, use of medical services, or health-related behavior. Under ideal circumstances, interview schedules are designed by experts in this activity, pretested on a small sample, and pilot-tested in the field to assess acceptability and validated by requestioning a sample of those already interviewed. In the highest-quality interview-based research studies, all interviewers are carefully trained to ask questions in a standardized manner and required to follow the procedures laid out in an interviewer's manual.
