In 1887 he became Librarian of the Royal Medical and Chirurgical Society, which amalgamated with others in 1906, becoming the Royal Society of Medicine. For the Library Association, he served as honorary secretary at its foundation (1887) and as president (1915–19). As editor of its periodical TheLibrary from 1889, MacAlister set high standards for the emerging profession of librarianship.
