The original hospital discharge abstract system, developed and implemented by the Commission on Professional and Hospital Activities in 1955, and adopted by many hospitals in the United States, Canada, and other countries. It tabulates data on diagnoses, procedures, length of hospital stay, and outcomes, yielding profiles of performance by hospitals, services, and individual attending physicians. It has been superseded by computerized medical records.
