A 36-item set of questions about health status and health behavior derived from the longer questionnaire used in the National Health Surveys conducted by the US National Center for Health Statistics. The SF36 assesses eight sets of variables: physical function, social function, role limitation, mental health, energy, vitality, pain, and self-perceived general health status.
