Incomplete provision of information about conditions for which data are required, especially in the interests of the public health. This is the bane of health information systems and of a comprehensive record of the status of many aspects of life. Underreporting of diseases, especially sexually transmitted diseases and to a lesser extent other communicable diseases, can occur for many reasons, for instance because the affected individuals never come into contact with anyone from the health care system or because health professionals do not identify the condition or fail to report the case to the public health information system. The consequence is distorted health statistics, and consequently inadequate planning and actions.
