This book explores a range of transitions that are commonly encountered by those likely to receive support from children's or adults' services. While some of these occur because of people moving across organisational boundaries, others are triggered as a result of decisions taken by individuals themselves. Following a chapter on the organisational context, the book examines the challenges of transitions for four particular groups of young people: those leaving care, those with learning disabilities, those with mental-health problems, and those seeking asylum. Other topics include the radical changes currently seeking to transform adult social services (from service provision to self-directed support), the transition to supported living for older people, and hospital discharge.
